Citrus, Tomatoes, and Acidic Foods: Do They Aggravate Hernias?

Popular foods in many diets include citrus fruits and tomatoes, but if you have a hiatal hernia, they are usually on the top of the list of foods to avoid. But do acidic foods really worsen a hernia, or do they just worsen the symptoms associated with it? Knowing the difference is important to manage your diet effectively. 

Acidic foods do not cause the hernia by themselves 

Acidic foods such as citrus and tomatoes don’t cause structural weaknesses that cause hiatal hernia. The hernia itself is caused by weakening of the diaphragm muscle – often due to age, increased abdominal pressure or genetic factors. It is not diet related. 

But they can make symptoms worse 

Acidic foods do play a large role in symptom severity. Once a hiatal hernia is present, the natural barrier that prevents stomach acid from traveling into the esophagus has already been broken. Adding highly acidic foods to the mix adds to the acidity that reaches the esophageal lining, which can exacerbate heartburn, regurgitation, and chest pain. 

Common acids to blame 

  • Lemons, limes, grapefruits and oranges 
  • Tomatoes and tomato-based products such as sauces, salsa, and ketchup. 
  • Vinegar-based dressings and pickles 
  • Certain fruit juices, even those marketed as healthy 

Why are some people more tolerant than others 

Sensitivity to acidic foods varies greatly from person to person. Some people with a small hiatal hernia have little trouble eating tomatoes or citrus, while others experience noticeable discomfort from even small amounts. It may be more useful to watch your own symptoms after eating than to follow a generic list of foods to avoid. 

Balancing Nutrition with Comfort 

Citrus fruits and tomatoes can be eliminated completely; they are important sources of vitamins and antioxidants. A lot of people find that they can reduce the symptoms by eating smaller portions, combining these with non-acidic foods or eating earlier in the day rather than being close to bedtime, while still having some dietary variety. 

When to Consider Evaluation 

If you experience frequent or more severe symptoms when eating acidic foods, this could mean your hiatal hernia has progressed or needs to be more closely monitored.
